$2.67B Blue Cross Blue Shield Class Action Settlement

All Local 21 members that were eligible for and enrolled in one of our Empire Blue Cross Blue Shield health plans (active or retiree) are eligible to participate in the $2.67 billion class action anti-trust litigation settlement. Post cards are beginning to be received by our members now, and you may also receive this communication via e-mail if you have provided such information to Empire or another Blue Cross Blue Shield health plan in the past. The postcards and/or e-mails contains a unique claimant ID that you can use to complete an expedited claim form, so please keep this for your records!
In order to participate, object or decline participation, you must go to the claim submission website (www.BCBSSettlement.com). Opt-outs and objections must be completed by July 28, 2021 and claim submissions to participate must be completed online or postmarked by November 5, 2021.
When you are filling out the claim form for coverage through Local 21, use the following information:

Plan Name: Empire HealthChoice Assurance, Inc. d/b/a Empire BlueCross BlueShield
Group #: Refer to your current/most recent Local 21 Empire ID Card
Subscriber/Member ID: Refer to your current/most recent Local 21 Empire ID Card
Coverage Start Date: When you first became eligible for health coverage through Local 21, or if later, February 2008.
Coverage End Date: When you last lost coverage, or if earlier, October 2020
PLEASE NOTE: Because this settlement covers such a long period of time back to February 2008, you MAY ALSO be eligible as a claimant if you were enrolled in another Blue Cross Blue Shield health plan outside of Local 21, such as through a prior employer or an insurance policy you purchased as an individual. When completing the claim form, you will need to provide as much information as possible about your enrollment in the Local 21 Empire Blue Cross Blue Shield health plan, but also will need to enter separate information about other Blue Cross Blue Shield health plans you may have participated with in the past (if applicable). If you did have coverage outside of Local 21, it is to your advantage to collect this information and include it in your claim filing.
